Please download and fill out the following forms and form packet in order to register your student for school.
Lindon Registration PacketPlease bring these required documents to the school when registering your child.
- Proof of residence or an approved exception
- A certified copy of the student’s birth certificate or other reliable proof of the student’s identity and age
- A certified and complete immunization record or official immunization exemption

If your child lives outside of Lindon Elementary boundaries, please click here to complete your out of boundary request. Your student will be assigned to their boundary school until their request has been approved.
